Rehab Rx Physical Therapy LLC is a cash-based physical therapy practice located at 545 Commerce St. Franklin Lakes, New Jersey, 07414. All services are provided on a direct-pay basis. We do not directly bill or contract with health insurance providers.
Patients are solely responsible for payment at the time of service. Upon request, we may provide an invoice (superbill) for you to submit to your insurance carrier for possible reimbursement, but we make no guarantees regarding coverage.
• The content on our website is provided for informational and educational purposes only.
• Nothing on the website should be considered medical advice or a substitute for consultation with a licensed healthcare provider.

• Payment is due at the time of service. We accept credit card, debit card, cash, Venmo, Zelle etc.
• Refunds are not provided for completed services. Prepaid packages or memberships may be subject to cancellation policies, which will be disclosed at the time of purchase.
• Appointments must be canceled or rescheduled at least 24 hours in advance.
• Missed appointments or late cancellations may be subject to a cancellation fee, at our discretion.
All content on our website, including text, images, logos, and graphics, is the property of Rehab Rx Physical Therapy LLC and protected under applicable copyright and trademark laws. You may not use, reproduce, or distribute our content without prior written permission.
